I hereby request the release of all completed test results arising from the laboratory services I have received from MNB Labor GmbH (the "Results"). I authorize the transmission of these Results to me via Livy Health GmbH, whom I have explicitly designated as my authorized representative for this purpose.

I confirm that I have been informed of and agree to the following:

(a) In accordance with applicable German law (§ 630g BGB), these Results may be transmitted to me electronically, and may also be made available to the physician who ordered the test(s) (“Ordering Physician”) either before or at the same time as I receive them;

(b) The transmission of the Results to me, or to Livy Health GmbH as my agent, does not constitute medical advice or diagnosis, and is not intended to replace a personal consultation with the Ordering Physician;

(c) Any questions or concerns regarding the interpretation, medical relevance, or recommended actions based on the Results must be directed to the Ordering Physician;

(d) The communication of the Results is solely for informational purposes and shall not include any personalized medical advice, nor shall it be interpreted as a form of compensation or inducement for medical referrals.

I understand that the Results include sensitive health data as defined under Article 9(1) GDPR, and I explicitly consent to their processing and transmission as outlined above, in accordance with Article 9(2)(a) GDPR. I further acknowledge that Livy Health GmbH is bound to handle these Results in compliance with applicable data protection laws, including Article 32 GDPR regarding data security, and Articles 44–49 GDPR if data is transferred outside the EU.
